Mail Completed Form(s) with check for Fee payable to Capital District FOP. If not using a photo we already have on file photo scan/email photo as attachment to info@fopalbany.com or mail us a hard copy of photo. If you are taking a new photo we need a solid color background. Finished ACTIVE Cards will have a red background, ASSOCIATE Cards will have a Blue background.
